南宮珉飾演羅以帝，他是泰江醫院應急醫學中心的精英，醫術高超，是一個不在意社會地位高低，絕不會忘記醫生和患者的緣分的充滿人性魅力的男人。金炳哲飾演善閔植，他是監-獄醫療課長，是一個有着精英選民意識的課長。他家族的醫療界族譜可以追溯到已經過世的爺爺那輩，父親的三兄弟也都是醫生，因爲父母的關係，大家都認爲他早就預定了大學教授之位，但其實並不是。雖然自尊心很強，但實力跟不上，總是落後於同期們。權娜拉飾演韓素錦，她是女子監-獄婦科負責人，是一名內科醫生兼修精神科。活潑開朗，爲了查明弟弟的死因而到監-獄醫院精神科當志願者。
